How to submit corrections and how we handle them.

Anyone can submit a correction with evidence. We review material corrections promptly, post a visible notice on the affected page, and preserve the historical record rather than silently overwriting it.

Submitting a correction

Send the specific claim, the correct information, and a primary source through the contact page. Provider and pharmacy corrections should include a first-party or official source.

How we respond

Material corrections trigger a visible dated notice on the page and an update to the underlying data record. Superseded figures are retained as Historical for transparency.
