What the trials establish
Compounded semaglutide and tirzepatide are prepared by pharmacies and were never the subject of the STEP or SURMOUNT trials. Those trials studied specific approved products manufactured to verified standards. A compounded product may contain the same molecule, but its formulation, concentration, and — for semaglutide — even its salt form can differ, and none of that has been tested for equivalence in a trial.
This means every efficacy or safety expectation for a compounded product is borrowed from the approved-product literature, not demonstrated for the compound itself. The borrowing may be reasonable in some respects and wrong in others; without a trial, we can't know which.
The evidentiary bottom line is stark and worth stating plainly: for compounded GLP-1 specifically, the randomized evidence base is empty. That is not an argument that it can't work — it's a precise statement of what is and isn't known.
